Organization Description

In 1971, a contingent of clergy and community activists formed Heights and Hills Community Council (now Heights and Hills) to direct attention to the growing needs of Brooklyn’s older adults. Today, Heights and Hills’ mission is to promote successful aging in the community. We do this by providing 19 Brooklyn neighborhoods a continuum of services that support healthy, independent aging.
We provide case management services to homebound older adults and their families in Boerum Hill, Brooklyn Heights, Carroll Gardens, Cobble Hill, Clinton Hill, Crown Heights, East Flatbush, Farragut, Fort Greene, Gowanus, Park Slope, Prospect Lefferts Gardens, Red Hook, Remsen Village, Sunset Park, Windsor Terrace and Wingate. We also provide information, education and supportive services to family caregivers in these same neighborhoods, as well as in Bedford Stuyvesant. As a supplement to the compassionate and professional work done by a multi-lingual staff ensuring the physical and emotional supports of homebound older adults, our Bridge-to-Home program that began in January 2013 serves the frailest of seniors who face multiple barriers to independent living post hospitalization and is a partnership with New York Methodist Hospital.
Other services offered include transportation and volunteer support through medical escorting, telephone reassurance and friendly visiting.
